CIK细胞在原发性肝癌三维适形放疗联合TACE治疗中的应用研究

摘要/Abstract

摘要： 目的评价细胞因子诱导的杀伤细胞（CIK）联合三维适形放疗和肝动脉化疗栓塞术（TACE）治疗原发性肝癌（PLC）患者的疗效。方法将51例PLC患者随机分为治疗组26例和对照组25例,分别接受三维适形放疗/TACE和CIK细胞回输或只接受三维适形放疗和TACE治疗。结果治疗组接受CIK细胞治疗后1月,外周血CD3+、CD4+、CD16+CD56+细胞百分比和CD4+/CD8+比值分别为（66.47±1.25）%、（56.98±2.65）%、（22.45±6.41）%和（1.52±2.10）,显著高于对照组的【（53.17±0.98）%、（48.09±0.27）%、（19.74±1.88）%和（1.13±0.06）,P<0.05】,而CD8+细胞百分比为（37.56±1.26）%,显著低于对照组的【（42.67±4.35）%,P<0.05】;治疗组生活质量改善率为84.6%,显著高于对照组的52.0%（P<0.05）;治疗组客观有效率为92.3%,显著高于对照组的68.0%（P<0.05）;治疗组1 a生存率为73.1%,显著高于对照组的52.1%（P<0.05）。结论 CIK细胞联合放疗和TACE治疗可以提高PLC患者的免疫功能,改善患者生活质量,提高近期疗效,延长生存期。

Abstract: Objective To evaluate the efficacy of cytokine-induced killer （CIK）,three-dimensional conformal radiotherapy（3D-CRT） and transcatheter arterial chemoembolization（TACE） in the treatment of patients with primary liver cancer （PLC）. Methods 51 patients with PLC were randomly divided into combination group （n=26）,receiving TACE/3D-CRT/CIK therapy,and control（n=25）,receiving TACE/3D-CRT therapy. Results At the end of one month therapy,the percentage of CD3+,CD4+,CD16+CD56+ and CD4+/CD8+ ratio in the combination group were （66.47±1.25） %,（56.98±2.65）%,（22.45±6.41） % and （1.52 ±2.10）,significantly higher than 【（53.17±0.98）%,（48.09±0.27）%,（19.74±1.88）% and（1.13±0.06）,P<0.05】,while that of CD8+ cells was（37.56±1.26）%,much lower than 【（42.67±4.35）%,P<0.05】 in the control;the improvement rate of life quality in combination group was 84.6%,significantly higher than that in the control group （52.0%,P<0.05）;the objective effective rate in the combination group was 92.3%,significantly higher than that in the control group（68.0%,P<0.05）;the 1-year survival rate in the combination group was 73.1%,significantly higher than that in the control group（52.1%,P<0.05）. Conclusion CIK cells combined radiotherapy and TACE for patients with PLC could improve the immune function,and patients' quality of life,which might lead to a good short-term efficacy and prolong the survival.
