干扰素-α治疗慢性乙型肝炎的疗效观察*

The effect of interferon-αon hepatic fibrosis in patients with chronic hepatitis B

摘要: 目的 探讨干扰素-α治疗慢性乙型肝炎患者对肝纤维化指标的影响。方法 50例慢性乙型肝炎患者接受干扰素α治疗6个月,检测血清肝纤维化指标和肝组织有关细胞因子表达的变化。结果 在治疗六个月后,37例(74%)患者HBV DNA转阴,27例(54%)发生HBeAg血清转换;血清肝纤维化指标明显下降(P<0.01),肝组织炎症活动度和纤维化程度明显减轻(P<0.01);肝组织转化生长因子β1、肿瘤坏死因子α、Fas、金属蛋白酶组织抑制因子1及I型胶原表达明显下降(P<0.01),基质金属蛋白酶1表达增强(P<0.05)。结论 干扰素α可能通过控制病毒载量能减轻免疫损伤,减轻肝脏炎症反应,抑制肝纤维化。

Abstract: Objective To investigate the expression of cytokines in the tissues of livers in patients with chronic hepatitis B receiving interferon-α therapy. Methods 50 patients with hepatitis B were treated with interferon-α for six months. The serum hepatic fibrosis index were assayed,and some cytokines were detected by immunohistochemistry in hepatic tissues before and after the treatment. Results After six month treatment,the HBV DNA loss was found in 37(74%),and HbeAg seroconversion was found in 27(54%)patients;the serum fibrosis index decreased,and the histological activity index and hepatic fibrosis improved;the hepatic expression of transforming growth factor-β1,tumor necrosis factor α,Fas,tissue inhibitor of metalloproteinase-1 and collogen I decreased(P<0.01),and the expression of matrix metalloproteinase-1 increased(P<0.05). Conclusion Interferon-α probably weakens the immunological injury of hepatocytes by decreasing hepatitis B virus load,alleviate inflammatory reaction and hepatic fibrosis.
