Abstract: Objective The aim of this study was to investigate the efficacy and safety of lenvatinib plus camrelizumab at base of transarterial chemoembolization (TACE) in dealing with patients with unresectable hepatocellular carcinoma (HCC). Methods 90 consecutive patients with unresectable HCC were encountered at Second Affiliated Hospital, Xi’an Jiaotong University between March 2019 and March 2022, 32 patients underwent TACE (T group), 30 received TACE in combination with Lenvatinib (T-L group), and 28 received TACE in combination with lenvatinib and camrelizumab (T-L-P group) therapy. Overall survival (OS), progression-free survival (PFS), Objective response rate (ORR), disease control rate (DCR) and adverse reactions (AEs) were compared, and factors affecting OS and PFS were evaluated by multivariate Logistic regression analysis. Results DCRs in T-L-P and T-L groups were 82.1% and 80.0%,both significantly higher than 50.0%(P<0.05)in T group, while ORRs in the three groups were 57.1%, 50.0% and 31.3%, without significant differences (P>0.05);median overall survival (mOS) in T-L-P group, T-L group and T group were 41.6 mon, 30.5 mon and 21.3 mon (P=0.002);mPFS in the three groups were 14.5 mon, 12.4 mon and 8.2 mon (P=0.001);multivariate Logistic regression analysis showed that T-L-P combination therapy was a factor which could prolong OS and PFS, while remote metastasis and vascular invasion were the risk factors impacting OS and PFS prolongation; there was no significant differences as respect to incidence of AEs in the three groups (P>0.05). Conclusion T-L-P combination is short-termly efficacious in dealing with patients with unrecectable HCC, with relatively safe in clinical practice.
