慢性乙型肝炎患者鼻内镜术后医用交联透明质酸钠凝胶填塞效果研究

Wound healing efficacy of cross-linked hyaluronan gel in patients with chronic hepatitis B and chronic rhinosinusitis undergoing endoscopic sinus surgery

摘要: 目的 研究医用交联透明质酸钠凝胶用于慢性乙型肝炎(CHB)合并慢性鼻窦炎(CRS)患者鼻内镜术后鼻腔填塞的效果。方法 2022年1月~2024年10月北京佑安医院收治的33例CHB合并双侧CRS伴或不伴鼻息肉患者,均行双侧鼻内窥镜手术。随机分组,在试验组鼻腔一侧采用医用透明质酸钠凝胶联合聚氨酯可降解膨胀止血绵填塞,在对照组则在另一侧鼻腔仅填塞聚氨酯可降解膨胀止血绵。术后门诊随访12周,行鼻内镜检查记录Lund-Kennedy评分系统 (LKES)评分、视觉模拟(VAS)症状评分和根据Boezaart 外科手术视野出血分级量表行鼻出血评分。结果 本组2例患者术后失访,31例患者完成常规术后随访;在术后第1周、2周、4周、8周和12周,试验组LKES 评分分别为(3.1±0.9)分、(2.5±1.0)分、( 2.0±1.0)分、 (1.6±1.0)分和(0.9±0.8)分,均显著低于对照组[分别为(4.2±1.3)分、(3.7±1.2)分、(3.2±1.2)分、(2.5±0.9)分和(1.5±0.9)分,P<0.05];在术后第4周、8周和12周,试验组粘连发生率分别为25.8%、32.2%和12.9%,均显著低于对照组的48.4%、58.1%和29.0%(P<0.05);在术后第3日、1周、2周、4周和8周,两侧鼻腔出血评分差异无统计学意义(P>0.05)。结论 采用医用交联透明质酸钠凝胶行鼻内镜术后鼻腔填塞能够有效促进CHB患者创面愈合,减少术后粘连的发生。

Abstract: Objective The aim of this study was to investigate wound healing efficacy of cross-linked hyaluronan gel in patients with chronic hepatitis B (CHB) and chronic rhinosinusitis (CRS) undergoing endoscopic sinus surgery. Methods A prospective,randomized controlled trial was conducted on 33 patients with CHB and bilateral CRS, with or without nasal polyps in Beijing You'an Hospital between January 2022 and October 2024, and all patients underwent bilateral endoscopic sinus surgery. For nasal packing after operation, patients were randomized to receive cross-linked hyaluronan gel along with degradable hemostatic sponge in one nasal cavity in observation group or to receivedegradable hemostatic sponge alone contralaterally in control. All patients were followed-up for 12 weeks postoperatively. Visual analogue scale (VAS) scores for symptoms, endoscopic sinus Lund-Kennedy scores and bleeding scores based on Boezaart criteria were recorded. Results Two patients were lost to follow-up in our series, left 31 patients ultimately completing routine follow-up; by week 1, 2, 4, 8 and 12, LKES scores in the observation group were (3.1±0.9), (2.5±1.0), ( 2.0±1.0), (1.6±1.0) and (0.9±0.8), all significantly lower than [(4.2±1.3), (3.7±1.2), (3.2±1.2), (2.5±0.9) and (1.5±0.9), respectively, P<0.05]in the control; by end of week 4, 8 and 12, incidences of local adhesion in the observation were 25.8%, 32.2% and 12.9%, all much lower than 48.4%, 58.1% and 29.0%(P<0.05) in the control group; by day 3, week 1, 2, 4 and 8 after operation, no statistically significant differences were found as respect to nasal bleeding scores(P>0.05)between the two groups. Conclusion We recommend the cross-linked hyaluronan gel for nasal packing in patients with CHB and CRS, which could promote rapid wound healing and reduce synechia after endoscopic sinus surgery.
