慢性乙型肝炎与乙型肝炎病毒携带者HLA和ApoE基因多态性差异分析*

Gene polymorphisms of HLA and ApoE in pathogenesis of chronic hepatitis B

摘要: 目的 分析人类白细胞抗原(HLA)和载脂蛋白E(ApoE)基因多态性与慢性乙型肝炎(CHB)发病的关系。方法 2021年4月~2024年4月我院诊治的79例CHB患者和100例慢性HBV携带者,采用直接测序法检测外周血HLA基因多态性,采用PCR法检测ApoE基因多重等位基因,根据文献进行基因分型。结果 CHB患者HLA-DQA1*0102和HLA-DRB1*15等位基因频率分别为13.9%和5.7%,显著低于HBV携带者的25.0%和14.5%,均具有显著性差异(P<0.05),而HLA-DQA1*0501、HLA-DQB1*0301、HLA-DRB1*03和HLA-DRB1*07等位基因频率分别为20.3%、31.7%、9.5%和15.8%,均显著高于HBV携带者的12.5%、20.5%、4.0%和7.5%,均具有显著性差异(P<0.05);CHB患者ApoE基因ε2/3基因型和ε2等位基因频率分别为15.2%和8.2%,显著高于HBV携带者的5.0%和3.0%,均具有显著性差异(P<0.05)。结论 CHB发病可能与HLA和ApoE基因多态性有关,携带HLA-DQA1*0102和HLA-DRB1*15等位基因可能对CHB的发生起抵制作用,而携带HLA-DQA1*0501、HLA-DQB1*0301、HLA-DRB1*03和HLA-DRB1*07等位基因或ApoE ε2等位基因可能起促进作用。

Abstract: Objective The aim of this study was to explore gene polymorphisms of human leukocyte antigen (HLA) and apolipoprotein E (ApoE)in pathogenesis of chronic hepatitis B (CHB) . Methods 79 patients with CHB and 100 chronic HBV carriers were enrolled in our hospital between April 2021 and April 2024, and peripheral bloodgene polymorphisms of HLA (HLA-DQA1, HLA-DQB1, HLA-DRB1)and ApoE were detected by direct gene sequencing. Results The frequencies of HLA-DQA1*0102 and HLA-DRB1*15 allelesin patients with CHB were 13.9% and 5.7%, significantly lower than 25.0% and 14.5% (P<0.05), while the frequencies of HLA-DQA1*0501, HLA-DQB1*0301, HLA-DRB1*03 and HLA-DRB1*07 alleleswere 20.3%, 31.7%, 9.5% and 15.8%, significantly higher than 12.5%, 20.5%, 4.0% and 7.5% in HBV carriers (P<0.05); the frequencies of ε2/3 genotype and ε2 allele of ApoE gene in patients with CHB were 15.2% and 8.2%, significantly higherthan 5.0% and 3.0% in HBV carriers (P<0.05). Conclusion The gene polymorphisms of HLA and ApoE are related to pathogenesis of chronic hepatitis B. Individuals carrying HLA-DQA1*0102 and HLA-DRB1*15 alleles might protect the infection from episode, while carrying HLA-DQA1*0501, HLA-DQB1*0301, HLA-DRB1*03 and HLA-DRB1*07 allelesand/or ApoE ε2 allele might have more chances to breakout.
